All PDSC campers should bring soccer cleats, soccer ball, shin guards, sunscreen and a water container. All equipment should be labeled with the camper’s name. A concession stand will be provided at all PDSC sessions. All snacks/refreshments will be priced at $1.00 or $2.00. The concession stand attendants will accept cash only for concession stand items.

Release Form: We, the parents of the above named child, do hereby grant permission for them to participate in the camp and acknowledge the fact that they are physically able to participate in camp activities. We understand that the camp does not provide medical insurance covering injuries of any nature incurred during the camp. This hereby releases all PDSC staff, coaches and Carroll ISD from any and all claims, demands, and causes of action whatsoever in any way growing out of or resulting from the participation in the PDSC camp.
