Liability release: In consideration of the forename’s participation in the Lady Dragon Softball Camp, I do hereby, for myself release and discharge all personnel, Camp Staff, Carroll Senior High School, and C.I.S.D. for all claims or other damages or other actions resulting from participation in the Lady Dragon Softball Camp. I have read and understand the information above. Refunds: Refunds will only be given when we receive written notice from a physician stating that the athlete is physically unable to participate in the camp. refunds will be reviewed by the sports camp administrator for validity. All approved refunds will incur a 15% administrative fee which will be deducted from your refund amount. please remember to bring glove, bat, helmet and water bottle [all with name of camper on it]. We will also have concessions available for purchase at the camp. You will receive a camp reminder in early June, prior to June 9th. Thank you! We are looking forward to it!

Release form: We, the parents of the above named child do hereby grant permission for her to participate in camp activities. We understand that the camp does not provide medical insurance covering injuries of any nature incurred during the camp. This hereby releases Tim Stuewe, the staff, and Carroll ISD from any and all claims, demands and causes of action whatsoever in any way growing out of or resulting from the participation in the softball camp.
