Liability release: In consideration of the forename’s participation in the Lady Dragon Softball Camp, I do hereby, for myself release and discharge all personnel, Camp Staff, Carroll Senior High School, and C.I.S.D. for all claims or other damages or other actions resulting from participation in the Lady Dragon Softball Camp. I have read and understand the information above.

Release form: We, the parents of the above named child do hereby grant permission for her to participate in camp activities. We understand that the camp does not provide medical insurance covering injuries of any nature incurred during the camp. This hereby releases Tim Stuewe, the staff, and Carroll ISD from any and all claims, demands and causes of action whatsoever in any way growing out of or resulting from the participation in the softball camp.
